There is a $10.00 fee for medical records retrieval. You can download a records release form from Here or from our website bannonvision.com and mail the form along with a $10.00 check to:

If only a copy of your eyeglasses or contact lenses prescription is needed, you can send your request to: info@bannonvision.com. There is no charge for a prescription.
